Inspector note: OBSERVED APPROXIMATELY 5 MOUSE DROPPINGS ON A LARGE BAG OF CORN STARCH IN THE REAR NEAR THE BACK DOOR, 25-30 MOUSE DROPPINGS ON THE SHELF BELOW THE PREP TABLE ON A COOKIE SHEET AND THE SHELF IN THE WEST PREP ROOM AND 7 UNDER THE 3 COMPARTMENT SINK. ALSO THERE IS NO PEST CONTROL LOG AVAILABLE DURING THE INSPECTION. INSTRUCTED TO CLEAN AND SANITIZE THE NOTED AREAS AND PROVIDE ADDITIONAL PEST CONTROL TO ELIMINIATE THE ACTIVITY AND PROVIDE AND MAINTAIN A PEST CONTROL LOG AT THE FACILITY. SERIOUS VIOLATION 7-38-020
