INSTRUCTED TO REMOVE CARBOARD. OBSERVED FOOD DEBRIS AND DARK COLORED STAINS IN THE POPCORN PREP AREA, ESPECIALLY IN THE CORNERS. INSTRUCTED TO CLEAN AND MAINTAIN. OBSERVED BROKEN, PARTIALLY MISSING FLOOR TILES IN FRONT OF THE POPCORN PREP AREA HAND SINK. ALSO OBSERVED LARGE, APPROX. 12 INCH LONG CHUNKS OF CONCRETE FLOORING DETACHED FROM THE BASEMENT FLOOR IN THE CENTER REAR PORTION. THE FLOOR UNDER THE BROKEN CHUNKS IS SOFT AND POROUS. INSTRUCTED TO REPAIR THE FLOOR IN THESE AREAS TO BE SMOOTH, DURABLE, NON-ABSORBENT AND EASILY CLEANABLE.

OF THE WALL. ALSO NOTED THE PAINT PEELING FROM THE WALL BEHIND THE 3 COMPARTMENT SINK IN A PATCH APPROX. 4 SQ. FT. INSTRUCTED TO FINISH THE WALLS AND MAINTAIN THEM TO BE SMOOTH, NON-ABSORBENT AND EASILY CLEANABLE.
