A DECAYED DEAD MOUSE UNDERNEATH A SHELF IN REAR LIQUOR STORAGE ROOM. FRONT ENTRANCE DOOR AND REAR EXIT DOOR ARE NOT RODENT PROOFED; MUST RODENT PROOF DOORS. MISSING DOCUMENTS NEEDED FOR PEST CONTROL LOG BOOK; MUST PROVIDE A COPY OF THE STATE OF ILLINOIS LICENSE, A MAP OF THE STORE, MSDS (MATERIAL SAFETY DATA SHEETS). ALL OF THE ABOVE MUST BE CORRECTED.
