Inspector note: Found previous serious violations not corrected. Original report date: 9/21/16, report # 1960908. #18 Found 20 live roaches the following areas in basement: under washer machine; on wiping cloths in bucket; on floor near ice machine;, on soda syrup boxes, in knife drawer and in separate storage room. Found live small black flies in the following areas: 5 in the women's and 7 in the men's restroom; 5 in customer seating area of bar; and 15 in bar area near sinks and soda guns. Found rat droppings in rear patio dining area in the following locations: 8 on the ground along the flower bed/fence; 60 in corners of rear of patio near grill (and along the alley side of the fence. Must remove droppings, clean and sanitize these areas. Eliminate fly and cockroach activity. Have premises serviced by pest control company. #29-Previous minor violation not corrected: #41 ELEVATE ALL STOCK OFF FLOOR IN BASEMENT STORAGE AREA. Critical violation 7-42-090.

Must repair and maintain. Found tiles with large gaps under hand sink in bar area. Must re-grout or repair. All surfaces must be smooth and easily cleanable. Found black, slimy mold like substance on floor of walk in, under compressor. Must remove, clean and sanitize. Must maintain all.
