    Inspector note: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Found front entrance/exit door not rodent proofed, door had 1/4 inch on bottom of door.Also found/observed rodent droppings(30 ) in front storage area shelving by pots, pans, also found rodent droppings(50 ) in rear storage aread under and on storage shelving, pallets. Must provide pest control log book, and missing docummentation (IDPH license, pest sighting log, location of applied pesticides etc). Citation issued # H000063593, with a court date of 2/17/20/11, at 400 w superior in room #112 at 9:00am. Must seal opening at door, remove all droppings and wash/sanitize surfaces.
