    Inspector note: OBSERVED EVIDENCE OF RODENT ACTIVITY AND LIVE INSECTS ON THE PREMISES. OBSERVED APPROXIMATELY 150 RAT DROPPINGS ON THE SHELVING UNITS IN THE STORAGE ROOM, BEHIND THE WALL AND ICE MACHINE IN THE SMALL PREP AREA BEHIND THE DELI AREA, AND IN THE SMALL ROOM NEXT TO THE STORAGE ROOM. OBSERVED APPROXIMATELY 150 MOUSE DROPPINGS ON THE STORAGE SHELVES BEHIND THE MERCHANDISE COUNTER, AND ON THE FOOD DISPLAY STORAGE SHELVES. MUST CLEAN ALL DROPPINGS AND SANITIZE ALL AFFECTED AREAS. ALSO, OBSERVED APPROXIMATELY 30 LIVE FRUIT FLIES AND 5 HOUSE FLIES IN THE FOOD PREP AND STORAGE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. ALSO, THE SIDE EXIT DOOR TO DAMEN AVENUE IS NOT RODENT PROOFED. MANAGEMENT INSTRUCTED TO PROPERLY RODENT PROOF THE SIDE EXIT DOOR. SERIOUS VIOLATION 7-38-020.
