    Inspector note: Evidence or rodent activity found on the premises. Observed mice dropping-approximately 35 behind clutter along south wallbase of basement dry storage,beneath the 1st floor grocery display shelvings.The exhaust fan in the basement has 1 inch gap all around unit,rear north side exit door must be tight fitting all across the side/bottom. Instructed to rodent proof door and exhaust fan in the basement to prevent rodent and insect entry. Also, establishment does have pest control logbook on premises. Instructed to remove all rodent activity and have licensed pest control service the premises and provide logbook with all required documents include an updated IDPH license,a map locator to identify the pest control services provided for this food establishment. Serious citation issued 7-38-020.
