    Inspector note: Observed potentially hazardous food at improper temperature inside the walk in cooler. Found eggs at 44.3-54 degrees F, found chicken at 55.4 degrees F. Instructed all potentially hazardous foods must be held at 40 degrees F or lower OR 140 degrees F or above, at all times. Operator voluntarily discarded all denatured foods during inspection. Operator estimates total value of 4# of discarded foods to be $55. Critical violation #7-38-005(a).

    Inspector note: upon investigation found rodent droppings in the following areas: aprox 35 droppings on floor under shelves in dry storage area on wall between drive through windows, aprox 25 droppings on floor of walk in cooler, aprox 115 droppings on floors throughout entirety of the basement. Ten live roaches were observed by the drain under the three compartment sink. 15 live ants were observed around a crack in the floor at the entrance of the employee break room. Instructed to remove droppings, clean and sanitize. Must eliminate pest issues. Advised to contact pest control company for service. serious violation #7-38-020

    Inspector note: Observed excessive grease buildup on the the side of the back rear fryer, and in between the sides on the top area of flat grills. Observed excessive food debris on the lower interior of the meat freezer in rear, and in the compartment next to soda fountain. Observed smoothie area covered in food debris & excessive spillage, in following areas: exterior of machine, surface counter of the machine, coffee area next to smoothie machine, and in fridge under smoothie machine. instructed to remove all excess debris, clean area sanitize. Must maintain status.

    Inspector note: Observed excessive open food, trash debris, and dirt accumulation on floor under and near shelves in storage area between the drive through windows. Observed excess food debris on floor of walk in freezer. Observed cardboard boxes used for storage open and directly placed on floor in storage area between the drive through windows. Observed excessive amount of grease on floor behind equipment where grease is stored in the basement. Instructed to remove debris, dirt, and grease. clean and sanitize. keep all products and equipment 6 inches above floor. Maintain status.

    Inspector note: Found several areas in basement with open ceiling/wall areas with pipes and beverage/grease lines going through them. Must seal or enclose all gaps in these areas to prevent rodent entry. Observed peeling paint/black slimy mold like substance on ceiling in hallway of basement. Instructed to remove the substance and paint. Clean and sanitize. Must repair ceiling to make it smooth and cleanable. Maintain status.
