Inspector note: OBSERVED ONE DEAD AND THREE LIVE ROACHES ON PREMISES. DEAD ROACH WAS ON THE FLOOR BY THE COOKING EQUIPMENT TWO LIVE ONES WERE ON THE FLOOR IN THE WASHROOM AND ONE LIVE ONE CRAWLING ON PREP TABLE IN REAR WHERE I WAS TYPING REPORT. INSTRUCTED TO CONTACT THE PEST CONTROL COMPANY FOR SERVICE. REAR EXIT DOOR LEADING TO OUTSIDE AND IS SHARED WITH SUBWAY RESTAURANT IS NOT COMPLETELY RODENT PROOFED AT BOTTOM; MUST RODENT PROOF DOOR. THIS DOOR LEADS TO OUTSIDE (SIDE) OF BUILDING THAT GOES TO THE DISPOSAL AREA.
