    Inspector note: OBSERVED APPROXIMATELY 35 RODENT MICE DROPPINGS ON THE PREMISES ON FLOOR IN CORNERS BENEATH THE CUSTOMERS DINING BOOTHS AND UNDER A COCA COLA COOLER BY THE BAR AREA AND NOTED APPROXIMATELY 20 RAT DROPPINGS SCATTERED ON FLOOR BEHIND CLUTTERED ARTICLES STORED IN THE BASEMENT ALONG THE NORTH WALL.THE FRONT DOOR IS NOT RODENT PROOFED 1/2" MODE OF ENTRY-MUST RODENT PROOF DOOR ALL ACCROSS THE BOTTOM.INSTRUCTED MANAGER TO DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS,REMOVE CLUTTERED CONDITIONS,SEAL ALL NOTICEABLE OPENINGS ON WALLS WERE NOTED,MAINTAIN FLOORS DRY IN THE BASEMENT TO CONTROL PEST PROBLEM, ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. ALSO LOG BOOK IS INCOMPLETE MUST PROVIDE MAP LOCATOR TO IDENTIFY PEST CONTROL SERVICES.SERIOUS VIOLATION 7-38-020.
