    Inspector note: No certified food manager on site while preparing and serving potentially hazardous foods such as chicken breast sandwich, meatballs, tuna salad etc. David Brooks with a sanitation certificate posted on the wall but it was expired dated 2/16/13. Instructed manager to re take the test for sanitation and be certified under City of Chicago. Citation issued serious 7-38-012.

    Inspector note: Must install a splash guard between the drain board of the 3 compartment sink and the prep sink. Caulk around the mop sink and the splash gaurd of the 3 compartment sink worn out with black moldy like substance. Must re caulk and maintain.
