INSTRUCTED TO REMOVE DEAD ROACHES FROM SAID AREAS AND CLEAN AFFECTED AREAS. • OBSERVED TWO ROACHES IN FACILITY, IN DINING AREA CRAWLING ON CEILING BEAM AND IN REAR HALLWAY NEAR EXIT CRAWLING ON CEILING NEAR PIPE. INSTRUCTED TO ELIMINATE ROACH ACTIVITY AND CLEAN AFFECTED AREAS. RECOMMEND PEST CONTROL SERVICE FACILITY ON OR AFTER 12/11/2023. PRIORITY FOUNDATION VIOLATION 7-38-020(A)
