LIVE FRUIT FLIES FOUND IN 2ND FLOOR BAKERY FRONT DRY STORAGE ROOM AREAS.INSTRUCTED MANAGER TO DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS,MAINTAIN FLOORS DRY IN THE FOOD PREP AREAS TO CONTROL PEST PROBLEM, ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY.ALSO,SEVERAL FRONT AND REAR EXIT DOORS TO THE FOOD ESTABLISHMENT,THE REAR ENCLOSED PATIO DOOR,2ND FLOOR BAKERY REAR EXIT DOOR,WALK-IN-COOLER DOORS NOT RINSECT/RODENT PROOFED 1/4" MODE OF PEST ENTRY WHERE NOTED TO MANAGER AT THIS TIME.A SCREEN IS NEEDED FOR THE TOILET ROOM DOOR WINDOW 2ND FLOOR BAKERY.A MAP LOCATOR INDICATING WHERE ALL THE PEST CONTROL SERVICES ARE PROVIDED MUST BE IN THE PEST CONTROL LOG BOOK.
