    Inspector note: OBSERVED LIVE FRUIT FLIES IN THE FRONT BAR AREA .APPROXIMATELY 10 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS, CEILING AND EQUIPMENT IN THIS AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. AND THE FRONT MAIN DOOR NEEDS REPAIR NOT CLOSING PROPERLY, IS NOT RODENT PROOF THERE IS A 1/4" GAP ALONG THE BOTTOM RIGHT CORNER A PEN IS ABLE TO FIT THROUGH SAID DOOR,2ND FLOOR EXIT DOOR MUST BE TIGHT FITTING ALL ACCROSS BOTTOM UNTIL NO DAYLIGHT SEEN.ALSO THE PEST CONTROL LOG BOOK IS INCOMPLETE FOUND NO IDPH COPY LICENSE INSIDE THE LOG BOOK AS REQUIRED. SERIOUS VIOLATION 7-38-020.
