FOUND REAR EXIT DOOR BOTTON NOT RODENT PROOFED.(ABOUT 1/2 INCH GAP). FOUND 2 WINDOWS AT 1ST FLOOR PREPARATION ABOVE DISHMACHINE WITH TORN WINDOW SCREENING. (7INCH HOLES) FURTHERMORE, FOUND SOME FLY ACTIVITY AT 1ST FLOOR/BASEMENT AREAS. OBSERVED ABOUT 20 FLIES. LAST, FOUND HOLES IN WALLS THAT MUST BE SEALED ACROSS WOK STATION AND UNDER WOK AREA AND TROUGHOUT PREMISES, A DOOR IS NEEDED IN BASEMENT AREA BY GAS METER. CRITICAL VIOLATION. CITATION ISSUED.
