    Inspector note: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments. 8 LIVE GERMAN COCKROACHES WERE FOUND IN THE GAPS IN THE SHEET METAL WALL AT THE SOUTH CORNER OF THE DISH ROOM WALL GAP AND BASEBOARD. 10 DEAD COCKROACHES WERE FOUND ON THE OTHER SIDE OF SAID WALL NEXT TO THE HOT WATER HEATER. REMOVE ALL EVIDENCE OF PEST ACTIVITY AND SEAL UP GAPS WHERE HARBORAGE IS OCCURRING.

THE FLOOR GOING OUT THE BACK DOOR IS GREASY, CLEAN AND MAINTAIN SAID FLOOR.
