-
Controlling Pests (Pf)Inspector note: During the inspection while in the dish area I observed a small rodent running across the floor and into a hole in the wall beneath the warewashing machine. The person in charge was able to provide a pest control report from 8-3-2026 which states there was activity observed in the form of deceased rodents and indicated there were structural voids beneath the back of house sinks that needed to be addressed. Have your pest control operator perform another service and as many as required to provide a clean IPM style report (no evidence no activity structural voids have been sealed and sanitation is acceptable) prior to the reinspection and send the report to isdhealth@boston.gov It is also highly suggested to increase frequency of services from your pest control operator until pest activity decreases or as recommended by your pest control operator.
-
(A) Certified Food Protection Manager (C)Inspector note: No Allergen Awareness certificate could be presented during the inspection; the person in charge signed up for the class and exam during the inspection; provide at least one person with a current Allergen Awareness certificate on staff.
