Controlling Pests (Pf)Inspector note: After looking over reports from 1/13/25 12/10/24 11/26/24 10/16/24 9/9/24 8/27/24. On 8/27/24 the exterminator stated in the report that all boxes need to be removed 6-18 inches from against the wall so that traps could be placed and proper cleaning could take place. This recommendation has been on every report since 8.27.25. At today's re inspection 1/21/25 the boxes have been placed on uboats and they were removed off of the left hand side wall but now they are up against the side wall. Please ensure that everything is18 inches off all of the walls and 6 inches off the floor. Kick plates have been removed from the chip aisle. Continue keeping this off. Ownership should consider purchasing shelving for the back room. To ensure all boxes are being stored properly off the floor. The exterminator needs to be able to put traps down and the floors need to be cleaned to help prevent with rodent activity. PIC stated that the pest company is due to come out tomorrow 1/22/25 because Monday was a holiday. Discontinue placing chips on the shelving over night. All chips need to be placed in totes and removed from shelving. Provide an action plan from the exterminator to ensure that the pest reports state no rodent activity. Recommend having a manager walk around with the exterminator to make sure that they understand exactly what corrective actions need to be taken to ensure pest can not get into the establishment. This part of the report also needs to be clean on the report. I spoke with 2 managers today and explained to them how important it is to look at the pest reports. The exterminator makes recommendations and if these recommendations are not being corrected then you will still have a pest problem. Boxes are still being placed on the floor and not on proper shelving. Discontinue using soda racks for storage.

Food Storage-Preventing Contamination from the Premises (C)Inspector note: Observed Uboats in the back room being stored up against the wall. Remove all uboats 6 inches 18 inches from the walls. Observed boxes still being stored on soda racks. Discontinue. Observed boxes in the break area being stored on the floor. Ensure they are 6 inches off the floor.

Floors Walls and Ceilings-Cleanability (C)Inspector note: Observed debis under the helium tanks and the bay next to that where all the shelving is stored on the floor. Clean/sanitize to remove debris. Observed items and debris on the floor in the back room. Clean/sanitize and remove all items off the floor.

Maintaining Premises Unnecessary Items and Litter (C)Inspector note: Observed over flowing trash in the dumpster outside. PIC stated that the trash was due to be picked up yesterday but due to the holiday they will be out today. Ensure trash is not over flowing out of the dumpster and that the lids are properly closed tight to help with rodent control. Clean to remove over flowing trash.
