  Inspector note: Observed rodent droppings near the front of the store next to the carts and in the back room near the break area and on the back wall. The store still has the chips stored on the 2nd shelf and the 1st shelf is empty. The kick plates are all removed from the shelving as well. Ecolab was coming out 2 to 4 times a month. The last report was 11/19/25. Please remove droppings from the floor and clean and sanitize the floors. Contact pest company and please provide a detailed IPM report. Report needs to be free and clear of all activity. No catches/no activity. Recommmend more frequent pest inspections.

  Inspector note: Observed missing lid on the dumpster. Replace and ensure that the lids are closed at all times. Observed trash and leaves on the grounds around the dumpster and container outside. At the time of the inspection staff member went outside and started cleaning up the grounds. Clean grounds to remove trash and debris.
