Controlling Pests (Pf)Inspector note: At the time of the re-inspection on (4/10/2025) there was evidence of what appears to be rodent droppings on the shelves in the front sales floor along with the back storage area's. Clean to remove and provide an additional pest control report for the re-inspection. Pest control is to provide a detailed IPM report depicting the current issues and the plan of attack for addressing the current rodent issue. Evidence of small flies observed in the back storage area. Address and provide a detailed IPM report.

All violations are to be addressed prior to the establishment being allowed to re-open. Establishment is not allowed to re-open until approval has been given by the Ciy of Boston Health/Buildings Departments.

Food Storage Prohibited Areas (C)Inspector note: Management is to ensure all products are being stored a minimum of 6 inches off the ground and away from the wall in order for pest control to be able to perform their inspections.

Nonfood Contact Surfaces (C)Inspector note: At the time of the re-inspection on (4/10/2025) there was an odor present upon entering the business and througout the store. Address and ensure all storage shelves are being cleaned and sanitized to help minimize any odors as well as rodent activity inside of the building. Back storage area is to be de-cluttered and all rodent droppings are to be removed and surfaces to be cleaned and sanitized. Front window sill visibly soiled and with debris. Clean and sanitize to remove all.

Floors Walls and Ceilings-Cleanability (C)Inspector note: At the time of the re-inspection shelves containing pet foods donuts paper products and back storage shelves visibly soiled and with what appears to be rodent droppings. Floor in the back storage room visibly soiled and with what appears to be old/newer rodent droppings. Clean and sanitize to remove all. (4/10/2025) • Side of building along the entrance of the store damaged (possibly due to unloading pallets of products) and with possible entry points. Address and make any necessary repairs.

Maintaining Premises Unnecessary Items and Litter (C)Inspector note: Back storage area filled with clutter and debris not allowing for pest control to address/access the current rodent issues. Management is to clear the back storage room of all clutter and debris in order for pest control to address and properly bait for rodents. All area's in the back storage room are to be completely cleaned and sanitized.
