System Maintained in Good Repair (P)criticalInspector note: Observed hose coming out of the brick wall near the trash area. Water is draining into the mop bucket. PIC stated the hose was coming from the AC unit. PIC stated that there was a water leak in the basement. No evidence of water at the time of the inspection. Observed leak under the 3 bays sink. Repair. Contact licensed plumber ensure to pull a plumbing permit at 1010 Mass ave for the repairs. Licensed plumber will need to contact the plumbing inspector after the work has been completed and then you will need to provide the signed off permit. Observed gang box (light switch) not installed properly in the womens bathroom. Repair Observed a hole in the floor in the women's bathroom. Repair/ Cap.

Cooling Heating and Holding Capacities-Equipment (Pf)Inspector note: Observed 2 refrigeration units that are not commerical grade refrigeration units. PIC stated the refrigeration unit downstairs is not working. Provide commerical grade refrigeration units. Email over the spec sheets for approval.

Controlling Pests (C)Inspector note: No evidence of rodents or fly's at this time. Exterminator was here on 8/19/25. Spoke to the pest company on the phone and they are sending over the report by the end of the day today. Please email over the report by the end of the day today.

Controlling Pests (Pf)Inspector note: Observed hanging fly catchers in the basement with a good amount of large black flies large black flies out back in the trash area and drain flies in the handsinks. Observed cochroach in the refrigeration unit downstairs in the basement that is not being used. Contact licensed exterminator and provide a detailed interior and exterior report. Please email the most recent pest report prior to next inspection. Remove fly catchers.

Nonfood Contact Surfaces (C)Inspector note: Observed heavy frost build up in the coffin freezer located in the basement. Defrost freezer and ensure the top of the freezer can close properly. Observed a soiled refrigeration unit located upstairs. Clean and sanitize interior and exterior of the refrigeration unit.

Cleaning Ventilation Systems Nuisance and Discharge Prohibition (C)Inspector note: Hood expired in May. Contact licensed hood company to inspect hood. Observed heavy grease build up on the hood filters and back splash. Clean to remove all grease.
