  Inspector note: Observed a dead mouse in the basement. Evidence of droppings in the basement. PIC provided me an pest report dated 7/12/24. He stated they come out once a month. Please remove dead mouse in the basement and clean and remove droppings. Contact licensed exterminator and provide a detailed IPM report for the re inspection.

Please take a photo of the updated hood sticker and send this over to ISDHEALTH@BOSTON.GOV after the hood has been inspected.
