Backflow Prevention (P)criticalInspector note: No air gap under the prep sink behind the sushi bar. Contact licensed plumber pull proper permit at 1010 Mass Ave. Plumbing inspector must approve before this report can be approved. At the time of the inspection the air gaps were on the prep sink but when you run the water it does not go into the base it ended up on the floor. Provide proper air gap.

Outer Openings Protected (C)Inspector note: No door sweep on the middle door at the bottom of the stairs. Provide door sweep that is flush to the ground and repair door to ensure is closes properly and no rodents can get through.

Conformance with Approved Procedures (P)criticalInspector note: Observed logs not being recorded. The last log was 3/16/26. Ensure staff is maintaining logs. Rice was left out last night and not stored properly in refrigeration unit. Rice- 76F. PIC removed rice and discarded at the time of the inspection. Ensure staff is disgarding rice at the end of the night or refrigerate over night at 41F or below. PIC was able to cook rice to test PH 4.0. Buffer solution expired in March- PIC was able to provide receipt. He purchased 4 days ago.

Controlling Pests (Pf)Inspector note: Observed 2 dead juvenile rats on a glue board in the basement. Evidence of rat droppings also in the basement. PIC was able to provide pest report 3/17/26. The report also states catches in the basement. The basement door at the bottom of the stairs leading to where the rodent activity is taking place was left open with no door sweep. Please consult with a licensed pest control/extermination company and obtain a detailed report confirming that there are no pest captures no signs of activity and no sanitation concerns. All identified or potential entry points and structural voids must be properly treated and sealed prior to the next inspection. Pest control service frequency should be increased as needed until the issue has been fully resolved and verified.
