Using a Handwashing Sink-Operation and Maintenance (Pf)Inspector note: Observed a box cutter inside the hand sink and other items being stored in the prep sink. Remove and clean and sanitize both sinks. Ensure staff is trained on proper storage.

Controlling Pests (Pf)Inspector note: Observed mice dropping in the back storage room next to the boiler. Clean to remove. PIC was able to provide a report from 3/20/26. No activity but the exterminator addresses the back door being left open that leads to the common area and sanitation issues. I observed the door being left opened. Please ensure the door is kept shut to ensure no rodent activity. Please consult with a licensed pest control/extermination company and obtain a detailed report confirming that there are no pest captures no signs of activity and no sanitation concerns. All identified or potential entry points and structural voids must be properly treated and sealed prior to the next inspection. Pest control service frequency should be increased as needed until the issue has been fully resolved and verified.

Clean organize and remove all items a minimum of 6 inches off the floor. Observed boxes and debris on the floor in the wall in refrigeration unit. Provide proper storage and clean floors.

Maintaining Premises Unnecessary Items and Litter (C)Inspector note: Observed ceiling tiles dishmachine and other items being stored in the common area hallway. Remove all items and clean and sanitize the floors to remove debris and grease.
