The IPM must address the steps required to deny the rodents access to the establishment the cleaning deficiencies identified and other steps that may be required to further mitigate the issue. The establishment should increase the frequency of treatment until later determined by the Health Division that rodents no longer represent an issue to the interior of the establishment. The ownership must provide a written log of the pests that are observed trapped and/or discarded.

    Inspector note: Modern Pest Services documented in their November 2013 and January 2014 reports that the hole behind the furnace needs to be patched. All stored goods chemicals and equipment in the rear dish room should be temporarily removed so that the walls and baseboard behind can be properly inspected for rodent access.
