    Inspector note: Observed the following previous serious violations not corrected from the last inspection on 6/13/18:Observed evidence of pest activity on site during inspection. Observed aprox 300 mouse droppings in the following areas: rear prep, front window ledge, cashier station, rear storage/employee personal belonging area, food storage shelving, dining area, and basement. Must remove droppings, clean, sanitize, and maintain areas. Facility advised to contact pest control provider for further service. Previous minor violations not corrected from inspection on 04/05/17. Violations state: "IDPH FOOD HANDLERS REQUIREMENTS NOT MET FOR EMPLOYEES.MUST ENROLL AND MAINTAIN, MUST REMOVE CARDBOARD FROM FLOOR UNDER COOKING EQUIPMENT IN TAQUERIA PREP AREA,MUST REMOVE PAPER FROM LOWER SHELVES OF PREP TABLES IN DELI PREP AREA,RUST FROM MEAT WALK IN COOLER STORAGE RACKS. MUST PROVIDE SPLASH GUARD AT HAND SINK NEXT TO 3- COMP SINK IN TAQUERIA PREP AREA. MUST ELIMINATE CRATES USED FOR FOOD STORAGE AND PROVIDE STORAGE SHELVES." A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Must correct violations.
