OBSERVED THE FLOOR DRAIN UNDERNEATH THE COOKING EQUIPMENT WITHOUT A COVER; INSTRUCTED TO PROVIDE A COVER FOR DRAIN. • INSTRUCTED TO CLEAN FLOORS UNDER AROUND AND BEHIND ALL KITCHEN EQUIPMENT AND IN AREAS WHERE RODENT ACTIVITY OBSERVED.
