All staff instructed to use 2nd hand sink on other side of cooking line until repairs made. Found restroom in kitchen not maintained. Restroom door locked and blocked by storage items (such as tables and boxes). Toilet has murky brown substance in bowl; water lines have been turned off to this room; observed excessive dirt and debris on floor of this room. Must repair and maintain all plumbing (sink and toilet) in restroom. Observed leak at pipes under 3 compartment sink in dish area. Observed leak under basin of the 2 compartment in banquet kitchen prep. Must repair and maintain.
