  Inspector note: Observed previous serious violation not corrected from report # 2176404, dated 5/29/18. Must complete the following previously issued serious violation: Observed approximately 45 mouse droppings in the following areas: on the floor in the front corner of prep next to hold holding area; on the floors under dry storage/bulk containers in rear of kitchen; on floors in restroom (in kitchen). Must remove all mouse droppings and contact pest control for service. Must clean and sanitize all affected areas. Observed approximately 75 mouse droppings on pipe in rear behind bulk containers; on the floor of the restroom; on the floor under the cooking line, under middle tables in kitchen; in area where bulk rice is stored; and on top of boxes of canned soda. Observe live mouse under middle tables in prep area. Must contact pest control for service. Must eliminate pest issue. Critical violation 7-42-090.
