OBSERVED THE FOLLOWING CROSS CONTAMINATION. INTERIOR OF ICE MACHINE IN BOTH UPPER WATER RESERVOIRS WITH BLACK AND PINK SLIME SUBSTANCE DRIPPING DOWN INTO ICE. ALL ICE DISCARDED. MACHINE TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL CLEANED, SANITIZED AND FAX REQUEST FOR RE-INSPECTION AND TAG REMOVAL OF ICE MACHINE TO 312-746-4240. OBSERVED APPROXIMATELY 5LBS COOKED PASTA IN A STRAINER IN THE KITCHEN MIDDLE COMPARTMENT OF THE THREE COMPARTMENT SINK WHILE THE OTHER WASH AND SANITIZE COMPARTMENTS WERE SET. DIRTY PANS OBSERVED IN THE WASH COMPARTMENT AND A WET, DIRTY TOWEL HANGING OVER SINK NOZZLE ABOVE THE CONTAINER OF PASTA. PASTA DISCARDED.
