ESTABLISHMENT WAS SERVICED ON 9/6/13 BY C&M PEST MANAGEMENT WHERE TECHNICIAN INDICATED NO ACTIVITY FOUND ON HIS REPORT; INSTRUCTED TO HAVE COMPANY SERVICE ESTABLISHMENT PRIOR TO REINSPECTION TO DETERMINE SOURCE OF ENTRY.
