Inspector note: OBSERVED A DEAD MOUSE ON A GLUEBOARD IN THE REAR STORAGE AREA UNDERNEATH A SHELF. ALSO OBSERVED SIXTY RODENT ACTIVITY (MOUSE DROPPINGS) ON THE FLOOR IN THE STORAGE AREA UNDERNEATH THE SHELVES, IN THE BUTCHER AREA ON THE FLOOR UNDER THE PREP TABLE, ON THE SHELF NEAR THE EXPOSED HANDSINK AND ON THE SHELF ON THE SELLING FLOOR IN THE FRONT AREA. MUST CLEAN ALL AFFECTED AREAS AND REMOVE DEAD MOUSE. SERIOUS VIOLATION 7-38-020 ISSUED.
