    Official wording: No Evidence Of Rodent Or Insect Outer Openings Protected/Rodent Proofed, A Written Log Shall Be Maintained Available To The Inspectors
    Inspector note: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments. Over 30 mouse droppings were found around and under shelving, safes and baseboards throughout the front counter area. Remove all evidence of pest activity and clean up rodent rub marks on foam sealer and wiring in all areas of possible pest harborage. Seal up the holes found around the pvc pipe used for the old soda lines and check the rear of the front cabinet and under the hot water heater for active rodent holes. All recent pest control records from Copesan Pest Control state "no activity found".

    Inspector note: All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Dirty food buildup was found in the interior portions of the center island, open up and wash, rinse and sanitize the the island top to bottom. The front counter cabinets are dirty and have old unused lock boxes that may harbor mice, remove old lock boxes and pest proof or remove said cabinets.

THE FLOORS AND BASE BOARDS ARE DIRTY THROUGHOUT THE PERIMETER OF THE FACILITY. MISSING AND LOOSE BASEBOARD TILES FOUND IN THE WALK IN COOLER, NEXT TO THE DRINK FOUNTAIN AND DISH ROOM. CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER. RESEAL THE FLOORS SO THEY ARE SMOOTH, SANITARY AND EASY TO MAINTAIN.
