    Inspector note: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES AND HOUSE FLIES INSIDE OF THE STORE. OBSERVED APPROXIMATELY 40 LIVE FRUIT FLIES ON THE WALLS, CEILING TILES AND BREAD RACKS IN THE BAKERY AREA AND ON THE WALLS AND CEILING IN THE DELI AREA. ALSO OBSERVED 5 HOUSE FLIES FLYING THROUGHOUT THE STORE. TECHNICIAN FROM MARK'S PEST CONTROL WAS ONSITE DURING THE INSPECTION. OBSERVED 2 FLY STRIPS HUNG FROM THE CEILING IN THE DELI PREP AREA. MANAGEMENT REMOVED THE FLY STRIPS UPON REQUEST. CLARK STREET RECEIVING DOOR IS NOT RODENT PROOF. OBSERVED A 1/2" GAP ALONG THE BOTTOM OF THE DOOR. MUST RODENT PROOF THE DOOR. SERIOUS VIOLATION 7-38-030.

    Inspector note: The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOORS OF THE SUSHI AND BAKERY PREP AREAS. DETAIL CLEAN THE FLOOR DRAINS IN THE BAKERY AND DELI AREAS TO ELIMINATE PEST HARBORAGE.

    Inspector note: The walls and ceilings shall be in good repair and easily cleaned. THE WALL SUPPORT NEXT TO THE DELI PREP TABLE IS NOT SMOOTH AND EASILY CLEANABLE. THERE ARE JAGGED EDGES AND CREVICES THAT CANNOT BE EASILY CLEANED. MUST COVER WITH A SMOOTH AND EASILY CLEANABLE SURFACE.
