Surface Characteristics-Outdoor Areas (C)Inspector note: Curbed areas surrounding restaurant with exposed soils under gravel that has been burrowed into by rodents per IPM pest control report from Waltham dated 4/5/22. Seal all suspect and potential areas with concrete after pest treatments and removals have been applied.

Outer Openings Protected (C)Inspector note: Voids in foundation on side of store and near trash storage enclosure. Seal all potential entry points on the perameter of the building with pest proof materials.

Removing Dead or Trapped Birds Insects Rodents and other Pest (C)Inspector note: Dead rat was observed extricated from the ceiling above the lobby per photographic evidence and as witnessed during inspection on 4.8.22. Remove any and all additional dead pests in ceiling and wall cavities after full assessment in conjunction with Burger King General Contractor and pest control provider. Noxious odors persist inside dining room and restrooms. Plumbing had been checked and was found in working order as of 4.8.22.

Repairing-Premises Structures Attachments and Fixtures-Methods (C)Inspector note: Persistent noxious pungent odors in dining room subject of constituent complaint on 4.8.22 still remains. Ceiling in lobby has since been removed and replaced with one dead rodent extricated. Ceiling in ladies room is slated to be removed and inside assessed for source of odors on 4.18.22 per on site PIC. Odors seem to be concentrated in the dining area and restrooms. Some odors apparent in kitchen office. Maintain kitchen office door closed. Make all necessary repairs. Determine source of longstanding odors and remediate with haste. Provide detailed plan of action and all applicable work orders to isdhealth@boston.gov ASAP.
