Nonfood Contact Surfaces (C)Inspector note: Persistent noxious pungent odors in dining room subject of constituent complaint on 4.8.22 still remains. Ceiling in lobby has since been removed and replaced with multiple dead rodents extricated per pest control company and contractor. Ceiling in ladies room is slated to be removed and inside assessed for source of odors on 4.18.22 per on site PIC. Odors seem to be concentrated in the dining area and restrooms. Some odors apparent in kitchen office. Maintain kitchen office door closed. Make all necessary repairs. Determine source of longstanding odors and remediate. Provide detailed plan of action and all applicable work orders to isdhealth@boston.gov ASAP. Clean and disinfect all interior wall and ceiling cavaties where rodent droppings are apparent. Upon re-inspection on 5.24.22 Odors still observed in the men's & womens rest rooms.PIC instructed to continue to provided additional remediation to remove all odors.PIC to provide all reports to ISDHEALTH@BOSTON.GOV

Removing Dead or Trapped Birds Insects Rodents and other Pest (C)Inspector note: Dead rat was observed extricated from the ceiling above the lobby per photographic evidence and as witnessed during inspection on 4.8.22. Remove any and all additional dead pests in ceiling and wall cavities after full assessment in conjunction with Burger King General Contractor and pest control provider. Noxious odors persist inside dining room and restrooms. Plumbing had been checked and was found in working order as of 4.8.22.
