Please ensure that the licensed plumber pulled the proper permit at 1010 Mass Ave. The licensed plumber will need to contact 1010 Mass Ave 5th floor plumbing division and schedule for the plumbing inspector to come out and inspect the air gap. This report can not be finalized unitl the plumbing inspector signs off.
