Food Storage-Preventing Contamination from the Premises (C)Inspector note: Observed multiple boxes being stored on the floor and food items being stored on the floor. Provide proper storage at minimum 6 inches off the floor. In the back room the shelving needs to be 6 inches off the floor . Remove bottom shelf and remove all card board boxes on the shelving. In the basement provide storage remove everything off the floor 6 inches away from the wall. Exterminator needs to be able to place traps. Remove all milk crates off the shelving in the store. 5-29-25 At this time PIC will need to be sure all product is store 6 inches off the floor in the retail and storage rooms. Remove all kick plates and be sure pest control is checking each inspection. Between inspection you will need to be sure it clean.

Nonfood-Contact Surfaces (C)Inspector note: As discussed the bulk head in basement will need to be closed off so no light is coming into the basement. Multiple traps at bottom of the stairs. Please install rubber around edges of the doors.

Maintaining Refuse Areas and Enclosures (C)Inspector note: Observed debris and leaves in and around the trash area. Clean grounds and ensure that the lids on the dumpser are closed at all times when you are not taking the trash out. Lids need to be locked and secure at night time. Remove all milk crates and soda racks. Remove all trash from inside and off the floor. Fill holes in the ground and cap off any piping outside. Remove any unnessary equipment hot boxes washer machine sofa air fryer ac machine coffin freezer. Clean the back rooms. Organize 5-29-25 PIC has cleaned up the back yard and covered the drain hole in the ground. Dumpster was empty and lids closed. PIC will still need to remove all personal cooking equipment old sofa and personal food in refrigerators.

Clean and organize behind counter. Remove medication and any chemicals and properly store in the back room.

Repairing-Premises Structures Attachments and Fixtures-Methods (C)Inspector note: As discussed PIC will need to remove the three bay sink that is not plumbed properly for commercial use and did not have drain boards. It was next to the mop sink and no hand sink available.

Controlling Pests (Pf)Inspector note: Evidence of mice droppings throughout the store in the back room and basement. Inside the bottom cabinets where the microwave is being stored on shelving in the corner behind cash register in the back room under the milk crates and on the shelving. Photos were taken. Contact licensed exterminator and provide a detailed IPM report provide a mass trapping provide more frequent visits from the exterminator. Have exterminator check the ceiling and outside the property. Clean and sanitize floors/shelves to remove all new and old mice droppings. Remove all day light from the bottom of the doors. Bulkhead side door leading to the trash area. Fill and seal all holes in the walls in the basement in the vents connect the piping in the basement. Remove any and all evidence of live cat in the basement.
