Controlling Pests (Pf)Inspector note: At this time found evidence of rodent droppings in the retail chip area. PIC will need to clean area and have pest control inspect. Pest control is inspecting a few days a weeks. On the reports did not see many catches. No kick plates and did not see any glue boards or boxes under shelves. Please address and email report to me. 7-12-24 New manager at this location. Please provide the last few pest control reports. Evidence found in the back room along the walls under shelves in retail and along walls in basement.

Miscellaneous Sources of Contamination (C)Inspector note: In retail area (chips shelf) found bags of chips that rodents have gotten into. PIC will need to ckeck each day and remove open bags. Suggested that bottom shelf be used for water and for pest control to put traps of some kind under shelf.

Nonfood-Contact Surfaces (C)Inspector note: PIC will need to clean retail shelves in food area. Evidence of rodent droppings and eat into bags of chips. 7-12-24 Please continue to clean shelves in retail area Provide a door sweep on doors in the back room.

Floors Walls and Ceilings-Cleanability (C)Inspector note: PIC will need to have the walls and the coving in the back room repired. Evidence of droppings along the wall. In the basement along the wall that would be the front of the store found multiple openings. Please block them off. • The floor under shelves back room and basement will need to be cleaned and report any holes or repairs needed to main office.

Maintaining Premises Unnecessary Items and Litter (C)Inspector note: The store will need to have a deep cleaning done and address all repairs to walls block the pipes behind the elevator and any other pipes going to basement. Extra shelves and other equipment should be store in a way that pest control can do a full inspection. Back room and basement will need to be cleaned up and organized.

Two racks of dairy items observed with elevated temperatures. Yogurt 70F half and half 69F. Person in charge indicated a work order has been placed. Repair.
