    Inspector note: Found two tubes of rice on counter. PIC stated that he just took it out of the refrigerator to reheat. Rice was between 56F and 61F. The rice was cooked the night before put in deep container and covered. PIC will need to review proper cooling with staff. I left a handout for staff and PIC to review. Rice was discarded by PIC

    Inspector note: PIC will need to review with staff proper cooling process cleaning of location proper sanitizing of equipment and wiping clothes. PIC will need to stay on top of general repairs and scheduled cleanings. Hand sink need to be stocked at all times so staff can properly wash hands between tasks. PIC will need to work woth pest control to help with issue. If pest control needs to come more often than every 15 day for a while to help then PIC should and work with then and ask what can be done to help.

    Inspector note: Found multiple dead roaches on basement floor on traps and some not. Behind cooking equipment found a trap filled with roaches. PIC stated that pest control inspects every 15 days. His last inspection was 4-11-14?? PIC will have to have pest control inspect again and removes all full traps and dead. Basement has a a lot of old not working equipment and other debris that needs to be remove.
