  Inspector note: Mouse droppings observed inside benches of main dining room. Mouse droppings also observed in multiple other dining rooms in this restaurant. Also observed dropings in the chemical room inside the heating vent covers basement storage areas coat room and host station closet. Provide extermionation service before the compliance inspection. Provide exterminators report with details of service and provide a game plan from exterminator for dealing with mouse issue. Also provide an intergrated pest management program from the exterminator that is specific to this restaurant. Have the reports and IPM report from the extermionator available at the time of the compliance inpection. Door sweep to kitchen door is broken on one end. ARepair or replace door sweep.

Stairs to basement nesar one of the back doore to this restaurant are soiled with heavy buildup of loose debris. Clean the staircase area. Remove all of the loose debris on stairs.
