Insects Rodents AnimalsInspector note: Mouse droppings observed inside benches of main dining room. Mouse droppings also observed in multiple other dining rooms in this restaurant. Also observed dropings in the chemical room inside the heating vent covers basement storage areas coat room and host station closet. Provide extermionation service before the compliance inspection. Provide exterminators report with details of service and provide a game plan from exterminator for dealing with mouse issue. Also provide an intergrated pest management program from the exterminator that is specific to this restaurant. Have the reports and IPM report from the extermionator available at the time of the compliance inpection. Door sweep to kitchen door is broken on one end. ARepair or replace door sweep.

Outside Storage Improperly MaintainedInspector note: At the outside trash area keep dumpsters closed at all times. If needed secure dumpsters with locks to ensure no interference.

Improper Maintenance of FloorsInspector note: flooor areas of the coat room host station and areas in the basmeent have heavy builduip of soils and clutter. Clean the floor areas and keepa ll supplies 6 inches off the floor. Stairs to basement nesar one of the back doore to this restaurant are soiled with heavy buildup of loose debris. Clean the staircase area. Remove all of the loose debris on stairs.

Also repair holes in baseboard of downstairs dining riooms. Replace missing ceiling tiles on ceiling at the employee entrance. Hole in ceiling of host station closet and hole in wall around pipe at the coat room. Repair and patch all visable holes in the ceiling.
