Backflow Prevention (P)criticalInspector note: No air gap on the prep sink in the kitchen. Contact licensed plumber ensure plumber pulls a permit at 1010 mass ave before this report can be signed off the plumbing inspector must sign off on the air gap. At today re inspection I noticed an air gap was installed with PVC. See notes above.

Controlling Pests (Pf)Inspector note: Exterminator was out earlier today waiting on pest report. PIC was able to send me March 16th and April 9th report. The reports did not have much on them. Spoke to the GM on the phone at today's re inspection and asked him to get us a detailed IPM report. There are 3 floors at this establishment. The basement has a very strong odor. There is a vent (photo taken) and the PIC stated that the exterminator does bait inside the vent. This is not on the extermination report. Please provide a detailed IPM report before next Friday's inspection. Please consult with licensed exterminination company and provide a report stating no catches/ no activity/no sanitation issues. Treat and seal any and all potential entry points and structural voids before the next inspection. Increase pest control visits as necessary until issue has been corrected. Continue to submit all reports via email for review. Continue to monitor all areas and clean and sanitize properly.

Repairing-Premises Structures Attachments and Fixtures-Methods (C)Inspector note: Observed ice machine on the 2nd floor. PIC stated it is not working due to a leak. Repair machine or remove. PIC stated that the plumber is scheduled to come out today. If the ice machine is not able to be repaired please replace or remove. Ensure all drains are capped off if you decide to remove it. Grease trap is being cleaned today. PIC and I spoke to the company on the phone at today inspeciton. Strong smell coming from the grease trap/dishmachine drain. Please have plumber look at the drain connected to the dishmachine.

Manual and Mechanical Warewashing Equipment Chemical Sanitization-Temperature pH Concentration and Hardness (Pf)Inspector note: Low dishmachine at the bar and low dishmachine in the kitchen area (2nd floor) sanitizer is low. PIC and I tried priming the the machines. Contact licensed company to educate PIC and repair the machines. Observed window cleaner stored at the waitress stand. PIC stated they clean the tables with. Provide chemical grade sanitizer for cleaning the tables.

Observed frost build up inside the small 2 door freezer next to the steam table. Remove foods defrost and clean unit. Observed heavy frost build up inside the walk in freezer in the basement. Defrost and remove. Organize walkin and properly store food items a minimum of 6 inches off the floor.
