Remove all the ice and clean and sanitize ice machine. Observed soiled counter behind the sushi bar. Remove all items off the counter and clean and sanitize counters.

Handwashing Cleanser Availability (Pf)Inspector note: No soap to clean hands at the hand sink next to the ice cream cooler and the hand sink behind the sushi bar. Provide soap at both hand sinks.

Hand Drying Provision (Pf)Inspector note: No means to dry hands at the hand sink next to the ice cream cooler. Provide means to dry hands.

Controlling Pests (Pf)Inspector note: Observed rat droppings on the shelving behind the susi bar on the floor under the hand sink behind the bar on the floor under the high temp dish machine. Observed cochroaches. PIC was able to provide a pest report from 9/25/26 that states no activity. Clean sanitize and remove any pest droppings. Please consult with a licensed pest control/extermination company and obtain a detailed report confirming that there are no pest captures no signs of activity and no sanitation concerns. Ensure that all identified or potential entry points and structural voids must be properly treated and sealed prior to the next inspection. Pest control service frequency should be increased as needed until the issue has been fully resolved and verified.

System Maintained in Good Repair (C)Inspector note: Observed a leak under the hand sink and slow drains at the the hand sinks behind the sushi counter. Observed a dripping faucet at the hand sink in the kitchen. Contact a licensed plumber to repair leaks and drains. Provide invoice.

Floors Walls and Ceilings-Cleanability (C)Inspector note: Observed rat droppings in the shelving behind the susi bar and on the floor under the hand sink behind the bar. Clean and sanitize to remove rat droppings. Observed soiled walls throughout the kitchen behind the 3 bay sink (remove old caulking and re caulk) behind the hot holding soup station Observed dust on the ceiling vent above the fliptop in the kitchen dust on both ceiling vents in the bathrooms. Remove and clean vents. Observed stained ceiliing tiles the hallway and soiled floors in the corner of the hallway. (See photo) Replace or paint ceiling tiles clean floor to remove soil. Observed Co2 tank in the hallway. Remove and ensure it is properly secured.
