Instructed facility to install and maintain a splash guard at the left side of the exposed hand sink to help prevent possible contamination. #34- Observed the basement floor to be littered with debris, trash, and dirt along walls and in multiple areas throughout. Instructed facility to elevate all items at least six inches off floor, remove debris and dirt, and clean and maintain floors. SERIOUS VIOLATION 7-42-090.
