Previous Serious Violation Corrected, 7-42-090criticalInspector note: Previou serious violation not corrected: 01-17-2013, report #1277660: #16-Foods not protected during storage, preparation, dipslay and service. Observed 15 trays of varieties ready-to-eat foods on top of display cooler not protected from possible customer contamination. Instructed to install shield or sneeze guard barrier between customers and food displayed. #18-Evidence of rodent activity found on premises. Observed 50 plus rodent droppings in basement scattered through=out. Instructed to remove all activity, detail clean area and have licensed pest control service premises. Must rodent proof basement rear exit door. Must provided completed pest control logbook with required documents from licensed pest control company including service invoice. #19-Outside garbage area not cleaned, maintained or rodent proofed. Outside outside garbage dumpster overflowing with garbage and debis and lids will not closed-elevated and not tight fitting. Also, observed numerous amounts of empty boxes, uncleaned utensils and storage containers, over 10-5 gallon containers of used cooking oil and food debris on ground outside of rear exit door leading to dumpster in alley. Instructed to detail clean area and have lids tight fitting to prevent rodent and insect entry. Must provided grease box for all waste grease disposal. Critical citation issued 7-42-090.
