Insects Rodents AnimalsInspector note: Heavy rodent activity noted in isles storing bags of chips. Address and provide an up to date detailed pest control report for the re-inspection. 8-28-17: Heavy rodent activity under shelving where chips were displayed remove shelving and throughlt clean and sainitze flooring area. 8-28-18: 7 bags of Dog food on shelving with holes eaten my mice manager discarded unsafe products at time of inspection. 8/29/17: Management is to provide a detailed IPM report to the City of Boston Health Department. Pest control services are to be provided on a daily basis and reports to be provided to the City of Boston. All kick plates are to removed throughout the retail portion of the store and store is to baited throughout. All storage area's and racks are to be accessible in order for pest control to monitor and bait appropriately. Carpets are to be cleaned by a professional company after pest control treatment has been completed with report provided to the City of Boston.
