Inspector note: Low temp Dishmachine in the kitchen Wash- 120F Rinse 110F. PIC contacted suburban and he was on site to repair. The probe was repaired. Tested the low temp dishmachine behind the bar and the test strips were blank. Observed chemicals. Suburban is repairing now. Ensure that the dishmachines are tested before opening.

Inspector note: Observed hot water tank leaking in the electrical room. Remove stagnant water and contact licensed plumber to inspect the tank- PIC stated he put in a work order for this and is waiting on parts. Provide invoice. Grease trap was repaired and a grate was inside causing it to over flow. Still observed a strong smell in the room where the sump pump and grease trap are located. Clean and sanitize to remove smell. Remove grate to help with the strong smell. Provide invoice for grease trap cleaning.

Inspector note: Observed a broken light shield over the dishmachine. Replace light shield with shatter proof cover. Observed smoke alarm in the womans bathroom hanging down. Repair. Observed ceiling tiles missing in the hallway next to the bathroom. Replace Observed soiled walls in the hallway downstairs debris under the stairs and soiled floors under the dishmachine. PIC observed at the time of the re inspection. Clean to remove soil. PIC stated at today's re inspection the electrictian is scheduled to come today to repair.
