(A)-(P) Person-In-Charge-Duties (Pf)Inspector note: I responded to the location for a report of rodents eating products in a counter display case.The manager on site has confirmed the incident and verified the video evidence that was sent to the Health division. The establishment was issued a TSOP(temporary suspension of the Permit) The Owner was instructed remain closed and to contact the Health Division to schedule a hearing.(617-635-5326) (ISDHealth@Boston.gov) The Owner was also instructed to contact the pest control company for service & email all reports to ISDHealth@Boston.gov. Manager was instructed to also provide any exclusionary work to prevent pest access. Manager is stating that an employee mistakingly left products out in a display case overnight. Manager instructed to re-train staff on closing procedures After hours contact numbers 311 or 617- 635 -4500 (Mayors hot line request Health On Call)

Controlling Pests (Pf)Inspector note: There was evidence of rodent activity observed in a counter display case after hours per a video sent to ISD Health Manager on site verified the incident.There is also evidence of fruit fly activity in the counter bakery display case(those items are display only)(Items have been removed & disposed).A small amount of mouse droppings were also observed in the dump sink lower cabinet Manager was instructed to contact pest control for service & provide all pest reports to ISDHealth@Boston.gov
