(A)-(P) Person-In-Charge-Duties (Pf)Inspector note: Report of a fire in the establishment Upon arrival I met with PIC Mike Millea who explained that a fire extinguisher had been knocked from its wall mount and discharged throughout the establishment The establishment was not operating at the time of the incident. Establishment instructed remain voluntarily closed PIC was instructed to provide a licensed professional cleaning company to clean/sanitize/disinfect all affected locations. PIC also instructed to provide all cleaning/sanitizing documentation to the Health Department be re-inspected before approval to re-open.
