    Inspector note: Sighted numerous live small flies through-out establishment. Live flies on utensils (mixing blades), walls, shelving/racks, floors and ceiling in prep area. Live flies on food/drink containers and walls in front display and service area. Flies also in basement hallway and washroom. Numerous flies flying in air also. Over 200 flies. Comments on Anderson pest control service report dated 7/27/12 state "Fruit flies are still heavy". Comments on 7/30/12 service report reads "fruit flies have improved very little...sanitation must improve." Also sighted live roach activity. Two live roaches on wall and one on container by prep area exposed hand sink. Must screen vent to outside at rear wall of prep area. Critical violation: 7-38-020. Must eliminate fly and roach activity. Have premises serviced by pest control company and comply with all following instructions regarding sanitation and maintenance of premises.
